Activity Coordinator

Description

Under the general supervision of the Senior Manager of Day Programs and in coordination with the Senior Coordinator, the Activity Coordinator is a full-time, frontline position responsible for the direct implementation, facilitation, and development of day program activities for persons with brain injuries.

The Activity Coordinator provides direct participant engagement through group, community-based, and individualized skill-practice activities, with virtual programming as an additional method of service delivery. . Activity and participant caseload assignments are made by the Senior Coordinator and may be adjusted based on participant needs, staff strengths, program priorities, attendance, and daily operational requirements. The position works collaboratively with all Day Program staff to ensure activities are meaningful, accessible, safe, person-centered, and connected to participant goals and the mission of Brain Injury Services. The Activity Coordinator is responsible for carrying out assigned activities and participant supports in accordance with program standards, established priorities, and organizational expectations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Activity Design, Facilitation & Collaboration

• Develop, prepare, and facilitate day program activities based on participant needs, interests, abilities, individual goals, program priorities, and established Standards of Practice.
• Lead and co-facilitate group and community-based activities, serving as lead or auxiliary facilitator as assigned by the Senior Coordinator. 
• Collaborate with other Activity Coordinators, the Senior Coordinator, and other program staff to plan, adapt, and implement meaningful, accessible, safe, and person-centered programming.
• Adjust activity responsibilities and facilitation approaches as participant needs, staffing, attendance, and program priorities change.
• Drives participants to and from community activity sites with Brain Injury Services’ passenger shuttle bus. 

2. Participant Engagement & Individual Supports

• Maintain an assigned participant caseload and provide individualized support through activity-based skill practice, individual meetings, and goal support; translate identified service needs into individual goals and service planning documents for assigned participants, as directed by the Senior Coordinator.
• Implement established individualized supports and Positive Behavioral Supports and communicate changes in participant needs or concerns.

3. Participant Documentation & Service Support

• Translate identified service needs into individual goals and service planning documents for assigned participants.
• Assist with assigned intake, assessment, orientation, transition, and service planning activities, and implement service decisions and supports for assigned participants in accordance with Standards of Practice.
• Complete assigned participant documentation accurately and in a timely manner in accordance with Brain Injury Services policies, procedures, and Standards of Practice.
• Support development and implementation of ISPs and individual goals as assigned.
• Assist with feedback collection, progress monitoring, and other program effectiveness measures.

4. Activity Supplies, Inventory & Resources

• Maintain the organization, availability, and readiness of activity supplies, materials, equipment, and other program resources.
• Monitor inventory and communicate activity supply, equipment, and resource needs through established procedures.
• Identify activity supply and resource needs and provide relevant information to the Senior Coordinator to support purchasing requests.
• Maintain clean, organized, safe, and activity-ready program spaces, Brain Injury Service shuttle bus, supplies, and equipment, and communicate maintenance or facility needs through Standards of Practice 

5. Safety, Team Participation & Program Improvement

• Respond appropriately to incidents and safety concerns, report them promptly through established procedures, and assist with required Brain Injury Service documentation and timely follow-up.
• Communicate participant, activity, operational, supply, and other program needs through established channels.
• Collaborate with program staff to support consistent service delivery and program continuity.
• Participate in staff meetings, training, professional development, quality improvement, organizational activities, and other responsibilities as assigned.

6. Other Responsibilities

• Perform other duties consistent with the scope and level of the position.

Requirements

Minimum Qualifications

• Bachelor’s degree in rehabilitation, human services, psychology, social work, special education, therapeutic recreation, library science, or a related field. Relevant equivalent education and progressively responsible experience may be considered if consistent with organizational policy.

• At least one year of experience providing community-based services to persons with disabilities, preferably persons with brain injuries.

• Experience or demonstrated ability in group facilitation, activity development, community-based services, or direct support of persons with disabilities.

Preferred Qualifications

• Experience in a community-based day program, brain injury rehabilitation program, Clubhouse, Day Program, or comparable human services setting.

• Experience using person-centered practices to support participant choice, independence, skill development, and community participation.

• Experience adapting activities to accommodate cognitive, physical, behavioral, communication, or other participant support needs.

• Experience facilitating community-based or recreational activities.

• Brain Injury Fundamentals training or Certified Brain Injury Specialist (CBIS) or Qualified Brain Injury Professional Specialist (QBISP) certification.

Required Training & Certifications

· Required training and certifications may be completed after hire within established agency timeframes.

· Maintain a current food handler or equivalent food safety certification recognized by the applicable local health department.

· Complete Brain Injury Fundamentals training within the established agency timeframe.

· Maintain current CPR/AED certification.

· Complete other agency-required training within established timeframes.

· Completes annual Shuttlebus Proficiency test and maintains Sure-Lok Vehicle restraint training certification  

WORK SCHEDULE

Monday through Friday, 8:00 a.m. to 4:00 p.m., unless otherwise arranged by the Senior Manager of Day Programs. Occasional evening or weekend hours may be required.

Cognitive and Decision-Making Demands

The position requires sustained attention, organization, communication, and sound judgment in a dynamic day program environment. Duties require the ability to prepare and facilitate activities, monitor participant engagement and safety, organize activity resources, identify participant and operational needs, adapt activities in real time, respond to behavioral or support needs, prioritize competing demands, and determine when consultation or escalation is appropriate.

Physical, Sensory, and Environmental Demands

This position works in a professional office, Day Program spaces, and community settings. It requires sustained engagement in an environment with frequent interruptions and varied sensory input; routine use of office, culinary, adaptive, and vocational technology equipment; local travel; and the ability to lift up to 40 pounds and perform activities such as bending, kneeling, standing, and using hand tools, with or without reasonable accommodation. This position may require local travel and, when assigned, operation of the Brain Injury Service passenger shuttlebus.
