Home Health Registered Nurse (RN)
One Home Health —Algonquin
One Home Health is looking for an experienced Registered Nurse to join our home health team. This isn't an entry-level role. We need someone who already knows how to manage a caseload, think on their feet in a patient's home, and work independently without a nurse's station down the hall.
What you'll do
- Manage a caseload of home health patients, completing skilled nursing visits, assessments, and care plan updates in the field
- Perform admissions, recertifications, and discharges, with accurate and timely OASIS documentation
- Coordinate with physicians, therapists, and the interdisciplinary team on changes in patient condition
- Educate patients and family caregivers on disease management, medications, and home safety
- Respond to changes in patient status and escalate appropriately
- Maintain documentation that meets Medicare, state, and agency compliance standards
What you need
- Active, unencumbered RN license in IL
- Minimum of [1-2] years of RN experience, with home health experience required
- Working knowledge of OASIS documentation and Medicare home health guidelines
- Comfortable working independently in patients' homes across [service area]
- Reliable transportation and valid driver's license
- Strong clinical judgment and the ability to prioritize a full caseload without close supervision
Preferred
- Experience with Axes
- Certification in wound care, oncology, or chronic disease management
