Are you the person your team turns to when they need a hand, a plan, or a calm voice on a busy day? Do you love working with seniors and helping caregivers do their best work? We’d love to meet you.

Amber Grove Place is looking for a Resident Care Coordinator (RCC) who brings compassion, follow-through, and a willingness to roll up their sleeves. In our memory care community, you’ll help keep resident care on track, support our caregiving team, and build relationships with the families who trust us with someone they love.

What you’ll do

Working alongside our Resident Care Director, you’ll:

• Lead by example. Coach, mentor, and support caregivers so they feel prepared to provide great care.
• Keep care personal. Participate in assessments and help develop and update care plans around each resident’s needs.
• Notice the little things. Recognize changes in residents’ health or behavior and communicate concerns promptly.
• Keep the team organized. Help manage caregiver schedules, assignments, and coverage across shifts.
• Be someone families can count on. Answer questions, share updates, and address concerns with patience and compassion.
• Connect the dots. Coordinate with hospice, home health, therapy, and other providers.
• Stay on top of the details. Maintain accurate records and help ensure care follows physician orders, service plans, safety practices, and California RCFE requirements.

Requirements

What you’ll bring

• Experience working with seniors, people living with dementia, or in a memory care setting is required (minimum 2 years experience).
• A high school diploma or equivalent. Additional education in healthcare, nursing, or a related field is a plus.
• Strong organization, communication, and problem-solving skills.
• The ability to stay calm when plans change and help others do the same.
• Comfort using computers for documentation and scheduling.
• Current CPR and First Aid certification, or willingness to obtain them.
• Ability to meet applicable California RCFE personnel, training, and clearance requirements.

Supervisory or leadership experience is strongly preferred. We’re looking for someone who can give clear direction, offer helpful feedback, and jump in alongside the team.

A little more about the role

This is an active, hands-on position in a memory care setting. Responsibilities include assisting with resident mobility and transfers and physical tasks involving up to 50 pounds, with reasonable accommodations as applicable.

The schedule may include evenings, weekends, holidays, and scheduled on-call responsibilities.
