What You’ll Do
As a Home Health Occupational Therapist, you will:

Perform comprehensive OT evaluations
-
Develop and update individualized plans of care
-
Assess functional limitations and barriers to independence
-
Address activities of daily living, including bathing, dressing, grooming, toileting, and meal preparation
-
Provide therapeutic activities to improve functional performance
-
Assess home safety and fall risks
-
Recommend adaptive equipment and modifications
-
Provide training in safe transfers and functional mobility
-
Develop home programs based on patient goals
-
Educate patients, families, and caregivers
-
Monitor progress and adjust treatment plans as appropriate
-
Complete reassessments and discharge planning
-
Collaborate with the interdisciplinary home health team
-
Complete accurate and timely documentation using WellSky
What We’re Looking For
-
Active Maryland Occupational Therapist License Required
-
Graduate of an accredited Occupational Therapy program
-
Reliable transportation
-
Strong clinical judgment and communication skills
-
Ability to work independently in patients’ homes
-
Excellent organizational and documentation skills
-
Home Health Experience Preferred, but Not Required
-
WellSky Experience is a Plus
