As a Hospice RN Clinical Team Specialist you’ll contribute to our success in the following ways:
- Applies professional nursing judgment to evaluate hospice documentation for clinical appropriateness, regulatory compliance, and eligibility requirements;
- Conducts a comprehensive clinical review of the initial plan of care and collaborates with clinical team to ensure all orders, goals, and interventions are present.
- Serves as a subject matter expert in electronic medical records (EMR), and hospice platforms, including SHP.
- Oversees workflow, including but not limited to admission, discharge, and HOPE.
- Provides clinical consultation, education, and mentoring to clinical team members regarding best practices and compliance with regulatory requirements.
- Collaborates effectively with members of the interdisciplinary team to support quality patient care and operational excellence.
- Utilizes operational data to identify systemic trends, develops recommendations and partners with leadership to implement process improvements
What is Required?
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
- Minimum of three to five (5) years’ relevant experience as a registered nurse in a Hospice environment; preferred experience in hospice quality activities.
- Current State License as a Registered Nurse.
- Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State.
- 10% travel.
- Meets all applicable agency policies and procedures related to health screening and required testing.
