Description
• Communicate effectively with residents, families, physicians, and interdisciplinary team members to ensure coordinated, person-centered care; promptly report significant clinical changes and update responsible parties.
• Administer medications and treatments as ordered; monitor resident responses, special equipment (oxygen, IVs, etc.), and implement appropriate interventions when needed.
• Develop, update, and evaluate individualized care plans; participate actively in care planning meetings and collaborate with interdisciplinary team members to support resident goals and care needs.
• Supervise and direct the daily work of LPNs, CMAs, CNAs, and non-certified nursing personnel ensure assignments are completed, care plans are followed, and documentation is accurate and timely.
• Provide orientation, guidance, and on-unit training to new and existing nursing staff; reinforce policy updates, best practices, and facility procedures.
• Initiate and conduct investigations related to resident or employee incidents/accidents and coordinate follow-up action as required.
• Perform specialty assessments such as skin, pain, hydration, or safety evaluations; update care plans and notify leadership and physicians of permanent or significant condition changes.
• Evaluate pharmacy indicators and collaborate with providers regarding medication issues, potential side effects, or concerns.
• Assume charge nurse responsibilities as assigned to support unit operations and ensure consistent quality of care.
• Participate in the facility Quality Improvement Program and required in-service education to maintain clinical competency and regulatory compliance.
• Perform other related duties as assigned to support resident care and facility operations.
Graduation from an accredited school of professional nursing (Associate Degree in Nursing or Bachelor of Science in Nursing). Prior experience in long-term care, acute care, or geriatric nursing preferred. Current licensure as a Registered Nurse in the State of Wisconsin required and maintained in good standing. CPR and First Aid certification required.
• Duties require frequent walking, standing, bending, reaching, pushing, and pulling throughout the shift.
• Must be able to lift, move, or reposition residents or equipment up to 50 pounds independently and more with assistance or mechanical lifting devices.
• Regularly assists with resident transfers and mobility using proper body mechanics and lifting equipment to reduce risk of injury.
• Requires sufficient visual, auditory, and sensory abilities to assess residents, monitor safety, operate equipment, and accurately document care.
• Work may involve exposure to infectious diseases, bloodborne pathogens, and other occupational risks; appropriate personal protective equipment (PPE) must be worn as required.
• May be exposed to variable temperatures, noise levels, and environmental conditions within resident rooms, hallways, and common areas.
• Work schedule may include days, evenings, nights, weekends, holidays, and on-call assignments based on facility needs.
• Risk of bloodborne pathogen exposure is considered high.
