Job Title: Home Health Aide (HHA) / Certified Nursing Assistant (CNA)

Reports To: Scheduling Coordinator and Nursing Supervisor

Approximately 19 hours currently available
Advise as 15–20 hours per week 

Position Summary:

The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) provides compassionate, client-centered personal care and supportive services to clients in their homes. Working under the supervision of the Nursing Supervisor and the direction of the Scheduling Coordinator, the HHA/CNA helps maintain the client’s comfort, safety, and dignity while promoting independence.

Key Responsibilities:

Client Care:

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, ambulation, and feeding.
• Provide assistance with prescribed exercises and mobility (as trained and permitted by regulation).
• Provide medication reminders as directed by nursing supervisor.
• Take and record vital signs, observe and report changes in client condition to the Nursing Supervisor as directed.
• Prepare meals and assist with feeding as needed.
• Perform light housekeeping to support a safe and clean home environment.
• Provide companionship and emotional support.

Documentation & Communication:

• Accurately document care provided and report changes in condition or concerns promptly.
• Use GPS/EVV app and company software for clocking in and out of shifts.
• Communicate regularly with the Scheduling Coordinator regarding assignments, availability, and scheduling needs.
• Follow the individualized care plan as directed by nursing staff.

Supportive Services:

• Provide companionship, socialization, and emotional support to clients.
• Assist with meal planning and preparation according to dietary guidelines.
• Perform light housekeeping duties such as laundry, dishwashing, vacuuming, dusting, and general tidying.
• Assist with errands, grocery shopping, and escort to appointments or social activities (as applicable).
• Provide reminders for medications(without administering), appointments, and hydration.

Client Safety & Communication:

• Observe and report changes in the client’s condition or home environment to the Nursing Supervisor.
• Communicate effectively with the Scheduling Coordinator regarding schedule changes, availability, and client concerns.
• Ensure the client’s living environment is safe and free from hazards.

Compliance & Conduct:

• Adhere to agency policies, procedures, and applicable state regulations.
• Maintain client confidentiality in accordance with HIPAA guidelines.
• Attend required training sessions and in-service education.
• Maintain professional boundaries and uphold the agency's code of conduct.

Requirements

Qualifications:

• High School Diploma or equivalent (preferred).
• Current Home Health Aide certification or Certified Nursing Assistant license in Massachusetts.
• CPR certification (current and valid).
• One (1) year of experience in a home care setting preferred.
• Excellent interpersonal skills and ability to work independently.
• Reliable transportation and willingness to travel within the service area.

Physical Requirements:

• Ability to stand, walk, bend, and lift for extended periods.
• On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift/carry up to 30 pounds.

Work Schedule:

• Flexible schedule; may include weekdays, weekends, evenings, and holidays depending on client needs and agency requirements.
