Position Overview
Breakthrough Healthcare is seeking responsible, dependable Home Health Aides (HHAs) who are confident working independently and committed to delivering high‑quality, compassionate care in the home setting.

Key Responsibilities
- Assist clients with activities of daily living (bathing, dressing, grooming, toileting)
- Provide mobility assistance and support with transfers
- Observe and report changes in client condition to the care team
- Maintain a clean, safe, and supportive home environment
- Offer companionship and emotional support
- Follow individualized care plans and agency policies
- Document care accurately and in a timely manner
- Uphold client privacy, safety, and dignity at all times
Qualifications
- Active HHA certification (or equivalent experience, where applicable)
- Prior experience in home care or facility‑based care preferred
- Ability to work independently and manage time effectively
- Strong communication and interpersonal skills
- Reliable transportation
- Commitment to professionalism and ethical caregiving
