Job Summary and Responsibilities
As our Care Coordinator RN, you will serve as a vital advocate for patients and families, ensuring the delivery of high-quality, cost-effective care. You will play a pivotal role in the Clinical Resource Management department by overseeing utilization review activities and managing the progression of care. By collaborating with physicians, nursing staff, and post-acute service providers, you will help navigate complex health needs while addressing cultural, language, and socioeconomic barriers to ensure optimal patient outcomes and an enhanced patient experience.
Every day you will perform comprehensive discharge planning assessments and facilitate multidisciplinary care plans that align with evidence-based clinical guidelines. You will actively engage with the healthcare team to manage inpatient resource utilization, identify readmission risks, and implement effective 30-day mitigation strategies. Your daily workflow will involve maintaining clear communication with stakeholders, delivering patient notifications, providing education, and documenting care transitions to ensure seamless continuity of services from the hospital to post-acute settings.

- Utilization Review & Care Progression: Perform rigorous utilization review activities and apply evidence-based clinical guidelines to manage inpatient resources and ensure patients receive the appropriate level of care.
- Multidisciplinary Discharge Planning: Facilitate and oversee the development and execution of comprehensive discharge plans by collaborating with the healthcare team, post-acute providers, and patients to ensure a safe, efficient transition.
- Readmission Risk Mitigation: Proactively identify patients at high risk for readmission and implement targeted, 30-day intervention strategies to improve long-term health outcomes.
- Patient Advocacy & Support: Serve as a dedicated advocate for patients and their families by identifying and addressing cultural, socioeconomic, or spiritual barriers to care and delivering essential patient education.
- Regulatory Compliance & Documentation: Ensure all care coordination activities, patient notifications, and transitions are documented accurately in accordance with hospital policies and Federal/State regulatory standards.
Job Requirements
Required -
Graduate of an accredited school of nursing upon hire and Minimum two (2) years of acute hospital clinical experience or
Masters of Case Management or Nursing field upon hire and 1 year of experience
- Registered Nurse: NE (RN:NE) upon hire and
- Basic Life Support - CPR (BLS-CPR) within 90 Days
Preferred -
- Bachelors Of Science Nursing upon hire and At least five (5) years of nursing experience.
- Certified Case Manager (CCM) N/A upon hire
- Accredited Case Manager (ACM) N/A upon hire
