Care Navigation services are available to adults or youth in Spokane County who are struggling to get or stay connected with healthcare providers.

Care navigators work one-on-one with individuals to find dental care, physical health care, and substance use treatment providers that are most accessible for them. Care navigators work as healthcare guides, helping to educate, advocate, and motivate individuals to meet their specific healthcare goals. Care navigators are able to provide assistance in numerous ways including:

  • Care coordination between multiple providers

  • Health promotion and advocacy

  • Finding providers to fit individual needs

  • Scheduling and appointment reminders

  • Transportation assistance (to and from appointments)

  • Referrals to community and social support services

The Supervisor leads, mentors and develops assigned employees by participating in employee selection and hiring, training, coaching and role modeling.  The Supervisor provides daily first-level problem solving, covers staffing gaps as needed and assists in resolving escalated customer concerns. The Supervisor guides, coaches and leads employees ensuring compliance with FBH policies, processes and practices.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  • Problem solves staffing issues related to sick calls and other absences.

  • Allocates work assignments and ensures workflows and processes are adhered to in order to maximize efficiency and partner agency support.

  • Conduct weekly or bi-weekly supervision sessions with each staff member assigned, keep a written log of supervision sessions.

  • Participate in interviewing and candidate selection for Care Navigators.

  • Collaborate with partner agencies in facilitating the needs and coordination requirements for shared clients.

  • Maintain identified workflows and assist in training and supporting Care Navigator team members in completing the required tasks.

  • Participate in Care Navigator team meetings and coordinate training, including on-boarding of new hires.

  • Demonstrate skill and knowledge related to large-programmatic functions and incorporate this into daily tasks of Care Navigators.

  • Provide back-up coverage to the Manager as needed.

  • Watch over large components within the program area and demonstrate effective communication skills in an effort to problem solve potential challenges.

  • Provide Care Navigation coverage at any FBH or primary care location in the Spokane area as requested

  • Demonstrate a positive attitude and effort toward meeting the needs of internal and external customers.

  • Must be able to apply active listening techniques conveying respect, professionalism, engagement skills, positive attitude and empathy.

  • Effectively utilize customer service and motivational interviewing skills to promote appropriate customer-centered care.

  • Demonstrate flexibility in responding to change and in working with diverse groups and individuals and a positive, solution-focused perspective towards organization goals and when working with co-workers and staff.

  • Support and encourage a harmonious working relationship within and between departments and work with assigned teams to promote a safe and welcoming environment.

  • Requires travel to see clients in the community and travel between work sites.

QUALIFIED APPLICANTS WILL HAVE:

REQUIRED:

  • High School Diploma or Equivalent.

  • Two years’ experience working in a behavioral health care or physical health care setting.

  • One year of management or leadership experience.

  • Must have an unrestricted and valid driver’s license, active car insurance and must be willing to use their personal vehicle in the course of work.

PREFERRED:

  • College education or specialty certification.

  • Customer Service experience.

KNOWLEDGE, SKILLS & ABILITIES:

  • Customer service experience and the application of active listening techniques are required.

  • Must have strong written and verbal communication skills.

  • Experience with basic computer software, including but not limited to: operating systems, word processing, email, internet and intranet is required.

PHYSICAL, SENSORY AND ENVIRONMENTAL:

  • Work is generally performed in an office setting which may involve sitting and use of computers for four to eight hours per day.

  • Blood-borne Pathogen Category IV - No exposure to blood-borne pathogens.

PAY RANGE (Hourly/ Non-Exempt): $26.92- $32.80

  • All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $26.92 per hour
