Victorian Post Acute is a 90-bed, rehabilitation-focused skilled nursing facility committed to helping residents achieve their highest level of independence and quality of life. We are currently seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our dynamic in-house therapy team.

Key Responsibilities
Evaluate residents and develop individualized treatment plans.
Provide skilled speech-language pathology services addressing speech, language, cognition, voice, and swallowing disorders.
Perform clinical swallowing assessments and implement dysphagia management programs.
Collaborate with physicians, nursing staff, dietitians, rehabilitation professionals, residents, and families to achieve optimal outcomes.
Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
Participate in interdisciplinary care conferences and discharge planning meetings.
Educate residents, caregivers, and staff on communication techniques, swallowing precautions, and home exercise programs.
Maintain compliance with all regulatory, facility, and documentation standards.
Qualifications
Current California Speech-Language Pathologist (SLP) license in good standing.
CCC-SLP preferred or eligibility as required by state regulations.
Strong communication, organizational, and interdisciplinary collaboration skills.
