A Physical Therapist (PT) is needed for a Full-Time Home Care position in Cleveland, OH. Enjoy a great work-life balance with a growing Home Care Company.
This full-time PT will cover Start of Care (SOC) and ongoing therapy visits across different areas, including the west side of Cleveland and Mentor.

Responsibilities for this position include, but are not limited to:
- Complete Start of Care (SOC) evaluations and ongoing therapy visits.
- Develop treatment plans by identifying a problem list and methods to achieve identified goals.
- Coordinate treatment plan with the patient, family, physician, and other team members.
- Adapt treatment to meet the individual needs of the patient.
- Review the quality and appropriateness of the total services delivered.
- Supervise PTAs and HHAs.
- Complete clinical documentation in a timely manner.
Requirements:
- Current state license to practice as a Physical Therapist in Ohio.
- Graduate of an accredited Physical Therapy program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Prior home care experience preferred.
- Strong communication, organizational, and time-management skills.
