Baptist Health is seeking to add a Care Coordination Assistant at our Baptist Metro campus. This is a full-time, day shift opportunity.
Under the direction of a licensed professional, the Care Coordination Assistant provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care. This role plays an important part in supporting smooth care transitions for patients and families.
Care Coordination Assistant, Job Responsibilities:
- Provide non-clinical and clerical support to the Care Coordination and Pediatric Social Work teams.
- Assist with coordination of care and discharge planning activities.
- Support patient and family care transitions across the continuum of care.
- Communicate with internal departments and external agencies as directed.
- Maintain accurate documentation and assist with data entry in the electronic medical record.
- Coordinate meetings, referrals, and follow-up appointments as needed.
- Ensure timely completion of assigned administrative tasks.
Care Coordination Assistant Experience:
- 1-2 years Hospital Experience Required
Education & Credential Requirements:
- High School Diploma required
- Basic Life Support (BLS)
- Driver's License
