Our Rehab at Home program is an established, evidence-based model that combines expert in-home physiotherapy, virtual care and multidisciplinary coordination to deliver exceptional rehabilitation outcomes beyond the traditional hospital setting.

Supporting patients recovering from elective orthopaedic surgery and acute private hospital admissions, the program enables safe early discharge, promotes faster recovery and reduces unnecessary hospital stays—allowing patients to rehabilitate in the comfort, familiarity and independence of their own homes.

About the Opportunity

We're seeking an experienced Grade 2 Physiotherapist to join our growing Rehab at Home team in Melbourne. This is an exciting opportunity to be part of an established, innovative service that is shaping the future of rehabilitation.Working autonomously across Melbourne, you'll become a trusted clinician for your patients while collaborating closely with an experienced multidisciplinary team.

You'll use your clinical expertise alongside digital health technologies to deliver contemporary, evidence-based rehabilitation that is personalised, proactive and centred around each patient's goals.

Key Responsibilities

  • Deliver comprehensive physiotherapy assessments and evidence-based rehabilitation programs within patients' homes.

  • Support patients recovering from elective orthopaedic surgery and acute private hospital admissions.

  • Develop personalised treatment plans that maximise recovery, mobility and independence.

  • Utilise virtual care and digital health technologies to enhance continuity of care and patient engagement.

  • Collaborate with medical, nursing and allied health professionals to optimise patient outcomes.

  • Support early supported discharge and hospital avoidance initiatives.

  • Maintain accurate clinical documentation and contribute to quality improvement activities.

  • Champion patient-centred, evidence-based rehabilitation practice.

About You

You're an experienced Physiotherapist who thrives in autonomous practice, enjoys building meaningful relationships with patients and is passionate about delivering exceptional rehabilitation outcomes.

You bring clinical excellence, sound judgement and a genuine commitment to helping people recover safely in their own homes.

You will bring:

  • Current registration as a Physiotherapist with AHPRA.

  • Minimum three (3) years' post-graduate physiotherapy experience (essential).

  • Experience in rehabilitation, orthopaedics, subacute care or community-based services (highly regarded).

  • Excellent clinical reasoning and evidence-based decision-making skills.

  • Outstanding communication, organisation and time management abilities.

  • Confidence working independently in community settings.

  • Experience using digital health platforms or telehealth (or a willingness to embrace new technologies).

  • Current Australian Driver's Licence.

  • Access to a reliable motor vehicle.
