Eligibility
- Active, employed team member in the Caregiver or LPN role at the end of each milestone month.
- In good standing (no active written discipline) at the time of each payout.
- Meets the attendance standard: no more than 3 call-offs or no-call/no-shows in the milestone month.
- Regularly scheduled part-time and full-time team members are eligible. Full-time team members receive the full amounts shown; part-time team members receive half of each amount.
How It Works
- The 6-month clock starts on the team member's enrollment date.
- Each milestone is paid on the first payroll after the milestone month closes.
- A missed milestone due to attendance or standing is forfeited. Later milestones can still be earned once the team member is back in good standing.
- Team members who resign or are separated before a milestone forfeit all unpaid amounts. Amounts already paid are not recovered.
- Approved leave (FMLA, medical) pauses the clock and resumes on return.
- Payouts are taxable wages and are processed through payroll.
Administration
- Tracking: Business Office / HR verifies eligibility monthly.
- Approval: Executive Director signs off before each payroll run.
- Communication: Each team member receives their milestone schedule at enrollment and a recognition note with every payout.
