Nurse Practitioner | Palliative Care

Location: Ocean County, New Jersey

A growing palliative care organization is seeking an experienced Nurse Practitioner to provide compassionate, high-quality care to patients with serious illnesses and complex chronic conditions throughout Ocean County and surrounding areas.

Responsibilities

• Conduct comprehensive palliative care assessments and follow-up visits

• Manage patients with serious illness and complex chronic conditions

• Facilitate goals-of-care conversations and advance care planning discussions

• Develop and implement individualized care plans

• Collaborate with physicians, specialists, hospitals, skilled nursing facilities, and community providers

• Provide symptom management and patient/family education

• Document patient encounters accurately and in a timely manner

• Participate in interdisciplinary team meetings and care coordination activities

Qualifications

• Active New Jersey Nurse Practitioner license

• National board certification and DEA registration

• Minimum of 2 years of Nurse Practitioner experience preferred

• Experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management preferred

• Strong communication and interpersonal skills

• Ability to work independently while collaborating within a multidisciplinary team

Preferred Qualifications

• Experience with home-based care, telehealth, or value-based care models

• Bilingual skills are a plus

Compensation & Total Rewards

• Competitive base salary: $130,000–$160,000 annually (open to discussion based on experience)

• Annual performance bonus of up to 12.5% of base salary, based on productivity (RVUs) and quality metrics
