POSITION SUMMARY  

The Manager, Patient Safety and Clinical Risk provides leadership for patient safety and risk management across CHEO. He or she is accountable to lead and build a program of activities to engage all of CHEO in enhancing patient safety and minimizing risk. The Manager, Patient Safety and Clinical Risk is responsible for planning, overseeing the development and implementation of the Patient Safety and Clinical Risk programming, including oversight of patient safety monitoring, assessment and care review and clinical risk management. The  

Program areas include but are not limited to management and oversight of Safety Reporting System, Care Review Committee and activities, Clinical Risk management, Accreditation and Solutions for patient safety culture and hosptial acquited conditions work.  

  

MAIN RESPONSIBILITIES  

  • Work with departmental leadership to develop a shared vision for Quality & Safety in alignment with corporate goals. 

  • Development and Implementation of Patient Safety and Clinical Risk programming for CHEO. 

  • Provide patient safety and risk management expertise and guidance to the Hospital. 

  • Ensures CHEO program-based quality and safety activities are in compliance with relevant legislation and corresponding regulations and accreditation requirements. Provides accreditation education to staff as required. Leads actions to respond to recommendations arising from accreditation surveys, and to address new accreditation requirements. Develop new policies and/or update existing policies as required to ensure compliance with legislation. 

  • Develop operating policies and procedures required for patient safety and risk management or for compliance with legislation.  

  • Act as a liaison and resource to departments and services within CHEO. 

  • Coordinate all aspects of Accreditation, the Safety Reporting System, Care Review, and Clinical Risk management.  

  • Leads retrospective (e.g Root Cause Analysis) and/or prospective reviews (e.g. Failure Modes Effect Analysis) necessitated by safety events, care incidents or process issues related to the program; monitors program staff and physician perceptions of safety, risk and quality improvement. 

  • Plan, coordinate and effectively manage resources (e.g. financial, human, physical space, equipment and materials) in alignment with the Hospital’s strategic plan  

  • Contributes to continuous quality improvement of the department by identifying and evaluating new initiatives, developments and trends in patient safety and risk management. 

  • Oversee Solutions for Patient Safety (SPS) work.

  • Other duties as assigned.

 

 QUALIFICATIONS  

  • Bachelors Degree (Essential) 
  • Master’s degree or PhD preferred (e.g. Health Administration, Leadership, Quality Improvement)   (Preferred) 
  • Regulated Health Professional in Ontario; Nursing background preferred (Essential)
  • Minimum five (5) years’ experience working in a hospital or clinical setting (Essential)
  • Minimum of five (5) years’ informally or formally leading teams or individuals (Essential)
  • Minimum of 3 years of clinical or program management experience. (Essential)
  • Experience working in a pediatric clinical setting (Preferred) 
  • Expertise in clinical patient safety science, Root Cause Analysis, and quality improvement science (Essential) 
  • Knowledge and understanding of Clinical Risk Management  (Essential) 
  • Knowledge of legal, ethical, and clinical issues associated with standard of care practices, and Accreditation Canada requirements.   (Essential) 
  • Experience in developing and implementing improvement program plans, including performance measurement (Essential) 
  • Ability to effectively inspire, lead, manage, motivate and mentor teams for patient safety and clinical risk (Essential) 
  • Ability to establish and maintain effective working relationships and work collaboratively with peers (Essential) 
  • Must communicate effectively and concisely, both orally and in writing (Essential) 
  • Must be able to deal effectively with difficult people and to handle difficult, stressful situations (Essential) 
  • Experience with Accreditation (Preferred) 
  • Bilingualism (French / English) (Preferred)  

 

*Experience and formal training combined with demonstrated performance and ability may substitute for stipulated academic/experience requirements

 Effective March 29, 2021, CHEO will conduct a more comprehensive pre-screening and evaluation process.  This process may include more varied and multidimensional testing such as a written test, practical exam, or other assessment. All candidates who meet the essential and preferred qualifications may be tested.  Bilingualism (French/English) will be validated by a language proficiency assessment conducted by a third party provider.
