• Full-time: $5,000
• Part-time: $2,500

Licensed Practical Nurse (LPN)

• Full-time: $3,500
• Part-time: $1,750

Starting pay is based on licensure, experience, qualifications, internal equity, and local market conditions. Additional earnings may be available through applicable shift differentials.

Chestnut Health Systems is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for our Residential Services team in Bloomington, Illinois.

 

Residential nursing gives you the opportunity to know the people you're caring for. Rather than seeing a patient for only a brief encounter, you'll be part of their care from admission through discharge, providing hands-on nursing, medication management, health education, assessment, and ongoing support.

 

You'll work closely with an interdisciplinary team of medical, behavioral health, and support professionals who communicate and collaborate to provide integrated, whole-person care.

Responsibilities

• Complete nursing assessments during admission and assist in developing initial plans of care.
• Support discharge planning to help ensure continuity of care after patients leave residential services.
• Carry out healthcare plans developed by practitioners.
• Administer prescribed medications and monitor patients for effectiveness, potential side effects, and other concerns.
• Assess patients in person for acute and ongoing physical health needs.
• Provide health education to patients and their families.
• Coordinate treatment with an interdisciplinary team that includes medical, behavioral health, and support professionals.
• Provide integrated service coordination to support whole-person care.
• Serve as a liaison among prescribers, pharmacies, other healthcare providers, patients, and members of the treatment team.
• Communicate changes in patient health or other concerns to appropriate members of the care team.
• Maintain accurate and timely documentation in the electronic medical record.
• Provide nursing support and guidance while helping maintain a safe, structured, and therapeutic environment.
• Protect the confidentiality and security of patient and organizational information.
• Promote Chestnut's culture of customer service excellence and uphold standards for quality patient care.
• Perform other duties as assigned.

Qualifications

Licensure & Education

• High school diploma or equivalent required.
• Active Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required.
• CPR certification required or ability to obtain certification within 90 days of hire.

Skills & Knowledge

• Knowledge of pharmacology and potential medication side effects.
• Strong nursing assessment and patient communication skills.
• Ability to provide clear and supportive health education to patients and families.
• Ability to collaborate effectively with prescribers, pharmacies, healthcare providers, behavioral health professionals, and other members of an interdisciplinary team.
• Basic computer skills, including experience with electronic medical record (EMR) systems.
• Strong organizational skills and attention to detail.
• Ability to provide compassionate, professional care to individuals with diverse backgrounds, experiences, and healthcare needs.

• Must successfully complete Chestnut's required background check process.
• Ability to work the schedule required for the position.
