Affirmed Home Care is seeking a compassionate, dependable, and skilled Licensed Practical Nurse (LPN) to join our team and provide high-quality, one-on-one care to a client in Newark, New Jersey.

What You’ll Do

• Provide compassionate nursing care according to the client’s individualized care plan

• Administer medications and prescribed treatments

• Monitor the client’s condition and promptly communicate any changes

• Provide wound, catheter, and other skilled nursing care within your scope of practice

• Maintain accurate and timely clinical documentation

• Promote the client’s comfort, safety, dignity, and independence

• Communicate professionally with the client, family members, and care team

What We’re Looking For

• Active New Jersey LPN License

• 1+ year of nursing experience preferred

• Current CPR/BLS certification

• Strong clinical, communication, and organizational skills

• Reliable, compassionate, and professional

• Comfortable working independently in a home care setting

• Valid driver’s license and reliable transportation preferred

• EHR/documentation experience preferred
