Family Services Coordinator III

Description

The Family Services Coordinator supports the mission, goals and values of Nevada Donor Network, Inc. (NDN) by screening and evaluating potential donors. The Family Services Coordinator is responsible for working with families to provide information regarding donation opportunities, obtaining authorization per NDN standards, conducting a comprehensive medical and social history, and providing bereavement support to families and hospital staff throughout the donation process. 

Requirements

Performs responsibilities of position and promotes teamwork and a professional working environment. 

· Responds to donor referral notifications via telephone within 20 minutes. 

· Responds on-site to all potential organ donors within 90 minutes unless prior approval is given by the Administrator on call to respond beyond 90 minutes. 

· Collects clinical information to determine donor suitability in collaboration with the Administrator on call. 

· Discusses donation options with potential donor next-of-kin or family with knowledge about organ, eye, and tissue donation per hospital guidelines.

· Works in close collaboration with hospital and other NDN staff to evaluate and determine the best time to offer families the opportunity for donation. 

· Assists donor families to accept death as described by the primary care physician. 

· Provides initial aftercare support, utilizes community resources to assist grieving families. 

· Provides referral responder coverage for hospitals and provides back-up as needed. 

· Ability to serve on a call schedule, with flexibility, for extended hours (can require day and night hours). 

· Work with families to obtain authorization for donation; obtain and document accurate med/social history.

· Assists with Donation after Cardiac Death (DCD) cases as assigned.

· Works with hospitals to increase hospital personnel responsiveness to potential organ and tissue donation cases by developing effective relationships with key personnel. 

· Assists in the coordination of Donor Remembrance Ceremony. 

· Collaborates with the Aftercare Coordinator and the Aftercare process. 

· Actively participates in hospital and public education programs as directed. 

· Assists with chart review and data collection as needed.

· Assists with survey/accreditation maintenance.

· Actively participates in donor council meetings.

· Actively participates in hospital education with a focus on palliative care and chaplaincy. 

· Leads the training and development of the Family Services department.

· Assists in the strategic planning process for the Family Services departments. Supports monitoring progress and completion of goals. 

· Actively participates in community education, and support through community service events. 

· Meets or exceeds performance metrics for NDN’s Family Services department. Actively participates in data analysis of the departmental performance and offers suggestions for improvement.

· Actively participates/volunteers at community service events hosted by NDN. 

· Collaborates and coordinates with other NDN departments.

· Serves as a Family Services AOC

· Assists in mentorship of new staff by coordinating and participating in orientation.  

· Other duties assigned. 

· Employees must adhere to and remain in full compliance with Nevada Donor Network’s Motor Vehicle Driving Policy as outlined in the Motor Vehicle Records and Driver Safety policy PC.334.
