Peninsula Post Acute is seeking a reliable, dependable, and compassionate Case Manager Assistant to join our team. Position Overview

The Case Manager Assistant provides administrative and care coordination support to the Case Management department, helping ensure residents receive timely services and a smooth transition throughout their stay and discharge process.

The ideal candidate is dependable, detail-oriented, professional, and committed to delivering excellent customer service while working collaboratively with residents, families, healthcare providers, and interdisciplinary team members.

Key Responsibilities

• Assist Case Managers with daily administrative and coordination duties.
• Communicate with residents, families, hospitals, healthcare providers, and community agencies.
• Support discharge planning and coordination of post-discharge services.
• Maintain accurate documentation and resident records.
• Assist with referrals, appointments, transportation arrangements, and follow-up services.
• Coordinate information between departments to ensure timely completion of tasks.
• Monitor outstanding items and follow through to completion.
• Maintain confidentiality and comply with HIPAA and facility policies.
• Provide professional, compassionate support to residents and their families.
• Perform additional duties as assigned to support the Case Management department.

Qualifications

• Previous experience in healthcare, skilled nursing, rehabilitation, social services, case management, or a related field preferred.
• Strong organizational and multitasking skills.
• Excellent communication and customer service abilities.
• Dependable with strong follow-through and attention to detail.
• Ability to prioritize responsibilities in a fast-paced environment.
• Proficient with computers and standard office applications.
• Ability to work effectively with residents, families, and interdisciplinary teams.
• Positive attitude, strong work ethic, and willingness to assist where needed.
