Blize Healthcare is dedicated to delivering high-quality, compassionate care to patients in the comfort of their homes. We are seeking a skilled and patient-focused LVN (IV Certified) to join our growing home health team.
Position Overview:
As an LVN, you will provide End of life Hospice Care, and skilled nursing care in a home health setting, working under the supervision of a Registered Nurse or physician. This role requires strong clinical skills, independence, and a compassionate approach to patient care.
Key Responsibilities:
- Perform comprehensive patient assessments and monitor ongoing progress.
- Develop, implement, and follow individualized care plans.
- Administer medications, including injections and IV therapy.
- Provide wound care, including dressing changes and monitoring for infections.
- Educate patients and their families about their conditions, medications, and care plans.
- Accurately document patient care, assessments, and progress notes.
- Observe and report changes in patient condition to supervising clinicians.
- Collaborate with interdisciplinary teams to ensure quality care.
Qualifications:
- Active LVN license (California).
- IV Certification (required).
- Valid BLS Certification.
- Minimum 1 year of nursing experience.
- Home Health or Hospice experience preferred.
- Strong skills in medication administration, wound care, and patient assessment.
- Excellent communication and interpersonal skills.
- Ability to work independently in a home health environment.
- Ability to lift up to 50 lbs. as needed.
- Valid Driver’s License and reliable transportation.
