Position Summary

The GIP Registered Nurse (RN) provides compassionate, skilled nursing care to patients receiving General Inpatient Hospice (GIP) services. The RN is responsible for assessing and managing complex symptoms, implementing individualized plans of care, coordinating with the interdisciplinary hospice team, and supporting patients and families during the end-of-life process.
The GIP RN provides clinically appropriate care focused on comfort, dignity, symptom management, and quality of life while maintaining the highest standards of nursing practice.

Essential Responsibilities

• Perform comprehensive nursing assessments of patients admitted for General Inpatient Hospice care.
• Assess, monitor, and manage complex or uncontrolled symptoms, including pain, respiratory distress, agitation, anxiety, nausea, and other end-of-life symptoms.
• Develop, implement, and evaluate individualized plans of care in collaboration with the hospice interdisciplinary team.
• Administer medications, treatments, and therapies as ordered, with particular attention to effective symptom and pain management.
• Monitor patient response to medications and interventions and communicate changes in condition promptly.
• Provide skilled nursing care appropriate to the patient's condition and goals of care.
• Educate and support patients and families regarding disease progression, symptom management, medications, and the dying process.
• Provide emotional support and compassionate presence to patients and families during end-of-life care.
• Collaborate with physicians, nurse practitioners, hospice aides, social workers, chaplains, and other members of the interdisciplinary team.
• Communicate significant changes in patient condition and care needs to the appropriate members of the care team.
• Maintain accurate, timely, and complete clinical documentation in accordance with organizational policies and regulatory requirements.
• Participate in interdisciplinary group meetings and contribute to ongoing evaluation of the patient's plan of care.
• Assist with transitions of care, including discharge from GIP when symptoms are stabilized or when the patient's level of care changes.
• Maintain patient safety, infection-control practices, and confidentiality at all times.
• Provide guidance and support to hospice aides, nursing staff, and other caregivers as appropriate.
• Follow all applicable hospice, nursing, facility, and regulatory standards.
• Perform other duties as assigned.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the state of practice.
• Graduate of an accredited nursing program.
• Current BLS certification.
• Hospice, palliative care, acute care, or comparable nursing experience preferred.
• Experience with pain and symptom management preferred.
• Strong clinical assessment, communication, and critical-thinking skills.
• Ability to work effectively with patients, families, physicians, and interdisciplinary team members.
• Demonstrated ability to provide compassionate, respectful, and patient-centered end-of-life care.
• Ability to remain calm and professional in emotionally sensitive and rapidly changing situations.

Preferred Qualifications

• Previous hospice or palliative care experience.
• CHPN certification or willingness to obtain certification.
• Experience caring for patients with complex symptom-management needs.
• Experience with IV medications, continuous infusions, respiratory symptom management, and other advanced comfort-care interventions.
• Knowledge of hospice philosophy, Medicare hospice requirements, and end-of-life care practices.
