What you’ll do:
The Transplant Coordinator oversees and coordinates the care of heart transplant patients throughout the pre- and post-transplant journey. This role serves as the primary liaison between patients, families, physicians, surgeons, and multidisciplinary care teams to ensure seamless evaluation, education, treatment planning, and ongoing support.
Responsibilities include managing transplant evaluations, reviewing diagnostic and laboratory results, coordinating patient care and medication management, maintaining transplant waitlist information with UNOS, and ensuring compliance with regulatory and insurance requirements. The coordinator educates patients and families on all aspects of the transplant process, provides clinical support and triage, facilitates communication among stakeholders, and supports quality improvement initiatives. This position also maintains accurate medical records, prepares reports and documentation for regulatory agencies, and collaborates across departments to deliver high-quality transplant care.

What you'll need for this job:
Minimum Education Required: Associates Degree
Preferred: Bachelor's Degree Specify Degree(s): Nursing
Minimum Experience Required: Required: 1-2 years of previous job-related experience
Preferred: 3-5 years of previous job-related experience
Details: Three years of professional nursing experience; at least 1 year of previous transplant, outpatient, or dialysis experience highly desirable
Licensure/Certifications:
Required:
- CPR (Cardiopulmonary Resuscitation)
- Current Registered Nurse License State of Illinois
Preferred: Specialty certification in clinical or functional area of nursing granted by a national nursing
organization.
Other: National Certification as Transplant Coordinator or other related specialty preferred.
Compensation:
Pay Range: $39.48- $65.14 per hour
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
