Community Advocate - Onsite (Harrisburg, PA)Job Description
This position requires the individual to work on-site at our office located in Harrisburg, PA.
The Community Advocate supports residents under Pennsylvania's Pharmaceutical Assistance Contract for the Elderly (PACE) Program by conducting initial intake assessments, documenting eligibility information, and facilitating appropriate handoffs for continued support. This role focuses on accurate data collection and delivering a clear introduction to available services.
Responsibilities
- Conduct initial intake assessments to gather basic client information and determine preliminary eligibility
- Document accurate and complete client information in accordance with established procedures
- Identify issues and escalate complex or unresolved cases
- Maintain organized documentation to support downstream service coordination
- Other duties as assigned
Minimum Qualifications
- High school diploma from an accredited school or equivalent GED
Must be eligible to work in the United States without the need for work visa or residency sponsorship
Additional Qualifications
- Clear verbal and written communication skills with an empathetic approach
- Ability to follow structured processes with attention to detail
- Strong organizational and time management skills
- Ability to work in a fast-paced, team-oriented environment
- Basic problem-solving skills with ability to escalate appropriately
Preferred Qualifications
- Previous health care, insurance, or pharmaceutical industry experience
