The multi-disciplinary Homeless Outreach Stabilization Team (HOST) will work in the community to provide services which includes substance abuse treatment, medical services, rehabilitative support, case management and peer support services.
The purpose of this program is to serve homeless individuals with behavioral health challenges who have demonstrated a combination of continuous service needs and functional impairments who have not shown to benefit from traditional outpatient programs currently available.
The Outreach and Engagement Specialist will work collaboratively with a multi-disciplinary team serving homeless adults experiencing severe and persistent mental illness and substance use disorders. Provides targeted outreach and engagement to individuals that are highly vulnerable, lack of service connections or other support, and unable to engage in traditional service models. Services will be provided in emergency homeless shelters, transitional homeless shelters, homeless encampments, transportation hubs, train stations, and other pertinent community location where people may gather.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Provide outreach and engagement services to homeless individuals with co-occurring disorders.
Utilize the Relational and Engagement Model (ROEM).
Provide consultation and training to homeless providers regarding target population.
Provide coordination and appropriate linkages to community resources and treatment agencies.
Assist individuals to find stabilization and transition services.
Coordinate with various mental health and chemical dependency treatment agencies, including DCRs, psychiatric and medical hospitals, and the criminal justice system.
Complete required documentation and data entry, including, if required, databases
outside of FBH such as the Community Management Information System (CMIS).
Participate in community meetings
Participate in Peer Review, supervision and intake and diagnostic staffing, in-FBH
educational and administrative meetings as directed.
Perform FBH HOST charting requirements, correspondence, HRIS and other
paperwork in a complete and timely manner.
Participate in psychiatric consultation, supervision, program meetings, and in-service trainings; participate in clinical reviews and case conferences for clients on caseload.
Keep abreast of community resources, networks and resources for the homeless population.
Requires travel to see clients in the community and travel between work sites either via FBH owned Vehicles and/or personal vehicle
Other duties as assigned.
QUALIFIED CANDIDATES WILL HAVE:
REQUIRED:
Bachelor’s degree in the behavioral health related field
Must apply for Agency Affiliated Counselor Registration within 30 days of hire.
Must have an unrestricted and valid driver’s license, be able to pass a motor vehicle screening and driving test, have active car insurance, and be willing to use their personal vehicle in the course of work.
PREFERRED:
2 years’ experience working in behavioral health care or health care setting.
Worked with psychiatrically impaired populations.
KNOWLEDGE, SKILLS & ABILITIES:
Ability to communicate and engage effectively with individuals of varied, backgrounds, trauma experiences, cultures, education level and socio-economic status.

This position provides client services in community settings on a daily basis. Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large.
Some environments may not be ideal in terms of cleanliness or being habitable.
Daily travel between locations is required.
The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.
Daily use of a computer is required.
May be required to lift 10 - 20 pounds.
Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.
PAY RANGE (Hourly/ Non-Exempt): $25.89-$31.53
All pay is based on longevity at Frontier Behavioral Health; thus the starting pay is $25.89 per hour.

Benefits described above are based on employees who hold .5 FTE positions or greater and/or some benefits vary based on FTE. PRN positions are not benefit eligible or eligible for a sign on bonus.
