Description
- Provides comprehensive, skilled physical therapy services/interventions in accordance with physician orders.
- Comfortable with start of care (SOC), Evaluations, Resumption of Care (ROC), Recertification (Recert), and Discharges (DC).
- Assesses patient skilled needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
- Collaborates with the interdisciplinary team to plan, evaluate team's goals for each individual patient.
- Educates, instructs, and trains patient, family, responsible caregiver on patient's physical therapy program.
Qualifications:
- Active/Registered by the Physical Therapy Board of California.
- Graduate from a Physical Therapy accredited school.
- Strong documentation skills.
- Excellent communication and interpersonal skills.
- Willingness to travel to patient locations for evaluations.
