Description
- Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
- Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
- Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
- Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
- Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
Qualifications:
- Active license/registration with the Physical Therapy Board of California.
- Graduate of an accredited Physical Therapy program.
- Strong documentation and organizational skills.
- Excellent communication and interpersonal abilities.
- Willingness to travel to patient homes for evaluations and treatment.
