Job Description Summary
The Health and Wellness Case Manager will provide nonclinical case management and primarily work within Student Affairs, addressing and responding to students of concern and providing health care resource management, wellness navigation, and holistic student support. The Health and Wellness Case Manager is responsible for serving as a key responder to students of concern through helping them identify, access, and coordinate care among appropriate on-campus and community resources.The Health and Wellness Case Manager as the university's primary wellness navigator and care coordinator, assisting students in accessing healthcare, mental health, telehealth, urgent care, specialist, and community-based resources. The Health and Wellness Case Manager will work collaboratively with students, families, faculty, staff, and community partners to provide support, facilitate resource connections, and promote student wellness.
The position will also be responsible for developing prevention programming, supporting students through health-related university processes, and assisting with collecting, analyzing, and reporting data for assessment needs and processes.
Job Description
Duties and Tasks:
Responsible for serving as a key facilitator to students needing non-clinical case management and referrals.
Point person for students who are hospitalized by facilitating resources and support such as transportation and access to prescriptions
Work effectively and compassionately with the families of students of concern within FERPA guidelines
Create and sustain programs and services that assist students of concern feel supported and connected to helping resources
Be an active, positive, contributing member of the department, the Division of Student Affairs and Saint Leo University
Outreach
Actively demonstrate sensitivity to the concerns and developmental needs of all Saint Leo students.
Assist residents with personal, social, and academic concerns through proper support and referrals.
Provide direction to students with different health and wellness related challenges
Will provide crisis response and de-escalation to students in distress in accordance with university policy and procedures
Develop and implement wellness education, prevention initiatives, and outreach programming that promote physical, emotional, social, spiritual, and academic wellbeing consistent with Saint Leo University's Benedictine values.
Coordinate and promote health and wellness resources through educational campaigns, workshops, outreach efforts, and collaborative campus initiatives.
Administrative
Monitors progress of students of concern and keeps comprehensive notes and records utilizing the Advocate CARE system.
Coordination of messaging of health services including written communication, social media, and marketing materials.
Serve as a competent and professional liaison to all offices and divisions of the University
Keep supervisors informed and up to date on all information and pertinent projects
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Demonstrate commitment to and experience in student support and programming
Ability to work within a diverse college environment
Ability to communicate, problem solve, and manage multiple priorities
Skilled at establishing rapport with students, staff, and internal and external partners
Ability to create trust and maintain confidentiality
EDUCATIONAL REQUIREMENTS / QUALIFICATIONS:
Bachelor's degree required in Social Work, Public Health, Health Education, Psychology, Human Services, Student Affairs, Nursing, or a related field.
One to three years of progressive experience in higher education, student support services, case management, healthcare navigation, wellness promotion, social services or related field
Proficiency with Microsoft Office and general office technology is required, along with the ability to learn institutional systems and databases
Any equivalent combination of education experience and training that provides the required knowledge, skills, and abilities
Physical Demands:
Will need to be able to stand for extended periods
Will need to be able to walk up/down multiple flights of stairs
