Description
- Deliver comprehensive PM&R care to geriatric and post-acute patients.
- Collaborate with interdisciplinary teams including nursing, therapy, and facility administration.
- Develop and implement individualized treatment and preventative care plans.
- Build strong relationships with patients, families, and facility staff.
- Lead initiatives to reduce hospitalizations and improve functional outcomes.
- Ensure continuity of care and clinical excellence across the facility.

Compensation & Benefits
