At Concho Hearts Hospice, our RN Case Managers have the opportunity to build meaningful relationships with patients and families while providing compassionate, one-on-one care wherever our patients call home.
We're looking for a caring and dependable Registered Nurse (RN) Case Manager to join our team serving patients throughout Wood County.
- Manage and coordinate care for an assigned caseload of hospice patients.
- Complete comprehensive nursing assessments and routine patient visits.
- Develop and update individualized plans of care.
- Monitor symptoms and provide pain and symptom management.
- Educate patients and families about medications, disease progression, comfort measures, and what to expect.
- Communicate changes in patient condition with physicians and the interdisciplinary team.
- Coordinate medications, DME, supplies, and other patient needs.
- Participate in interdisciplinary team (IDT) meetings.
- Provide emotional support and guidance to patients and caregivers.
- Complete accurate and timely documentation in the EMR.
- Collaborate with LVNs, CNAs, Social Workers, Chaplains, Physicians, and other members of the hospice team.
- Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
- Strong nursing assessment and critical-thinking skills.
- Hospice, home health, skilled nursing, or acute-care experience preferred.
- Compassionate and confident communicating with patients and families.
- Strong organizational and time-management skills.
- Ability to work independently while collaborating with the interdisciplinary team.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to travel throughout the Wood County service area.
