Position Overview: The Community Health Coordinator (CHC) is responsible for supporting clients through care transitions, promoting health and wellness, addressing social determinants of health, and connecting individuals with medical and community resources.
Position Responsibilities: The CHC will assist clients in identifying personal health goals that promote independence, maintain accurate documentation within required timeframe. Conduct outreach following hospital, emergency department, rehabilitation, or skilled nursing facility discharge. Complete food insecurity and health-related social needs screenings. Connect clients with Food is Medicine programs and nutrition resources. Promote preventive care and routine follow-up with healthcare providers.
Qualifications:
- An Associates Degree with at least one year professional experience in the field or
- A Registered Medical Assistant with 2 years of experience
- Bilingual preferred
- Experience in primary care, hospitals, urgent care, rehabilitation, home health, skilled nursing, care coordination, or community health.
- Experience working with individuals living with chronic medical conditions.
- Excellent communication skills with the ability to establish and maintain positive and professional relationships with team members, consumers and their support systems, providers and business partners.
- Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook
- Effetive documentation skills with the ability to complete documentation that is timely, accurate and comprehensive (for HC CM)
- Ability to process information accurately and make decisions based on the specific situation and circumstances
- Ability to work both independently and as part of a team
- Demonstrates strong work ethic, dependability and commitment to consistently meeting responsibilities and deadlines
- Professional demeanor and ability to handle confidential information and documents with discretion and according to GLSS policy and HIPPA guidelines and regulations
- Valid driver's license and reliable transportation with the ability to conduct home visits, attend community events or conduct other GLSS business throughout the GLSS service area
Hours: Full Time 40 hours per week (Monday-Friday). Hybrid, requires one-two days per week in the office. Community visits, home visits, and in-office meetings may be required as business needs dictate.
Page Range: $22.50-$24.50 per hour
