What to Expect: 
Responsible for planning, providing, arranging, and monitoring services to consumers participating in Comprehensive Community Services (CCS), Coordinated Services Team (CST) program, and to consumers who are dually= enrolled in Childers’s Long Term Services. (CLTS) under the close supervision of the Division of Community Programs Program Supervisor.
Essential Duties:
  • Perform clinical assessments including initial intake and yearly reviews.
  • Develop, direct and implement treatment plans, interventions, and crisis plans using a trauma-informed and person-centered approach.
  • Ensure that the member receives culturally, linguistically, and developmentally appropriate and timely manner.
  • Provide information, outreach, and assistance to youth and their families.
  • Refer consumer to contracted services and monitor to ensure the consumer can achieve the highest level of independent functioning.
  • Provide individual and family psychoeducation and individual skill development in community-based and home settings.
  • Encourage investment and participation of natural supports in recovery concepts and processes.
  • Interface with healthcare providers and other social service agencies to coordinate services.
  • Assist families with transition to adult services and connect them to resources related to needs.
  • Function as part of treatment teams/multi-disciplinary teams to ensure continuity of care.
  • Communicate effectively through verbal, non-verbal, and written communications.
  • Continually seek to improve skills and demonstrates a high level of self-direction. 
  • Maintain accurate and timely documentation in the form of assessments, treatment plans, progress notes, etc. as required by agency policy and State statute requirements including the authorizing and distribution of funds when warranted.
  • Attend and participate in agency staffing and in-services. 
  • Participate in one on one, side by side, and group supervision.
  • Other duties as assigned, including serving in a coverage role for other department staff.
Qualifications:
  • A minimum of a bachelor’s degree in behavioral science and 1,000 hours of supervised post-degree clinical experience serving individuals with severe and persistent mental illness, or a bachelor’s degree in a field other than behavioral science with 2,000 hours of supervised post degree clinical experience serving persons with severe and persistent mental illness preferred.
  • Knowledge of or experience with trauma-informed principles and practices is preferred.
  • Knowledge and understanding of federal laws, State statutes, and administrative rules which regulate the services to be provided.
  • Knowledge and understanding of the programs, funds and services available.
  • Knowledge of and sensitivity to social problems, cultural diversity, human growth and development, and individual and family functioning.
  • Ability to coordinate activities and functions that cross unit and departmental lines and involve other county and community agencies.
  • Ability to establish and maintain effective working relationships with coworkers, superiors, community partners, and the public at large.
  • Ability to relate to people and to communicate effectively in both written and oral formats.
  • Ability to maintain confidentiality.
  • Ability to exercise sound judgment in confrontational and stressful situations.
  • Ability to follow instructions, to complete tasks, and to work closely with the supervisor and others.
  • Must have unlimited access to a car, a valid driver's license, automobile insurance, and the ability to efficiently travel in and out of county, as needed.
Work Environment & Physical Requirements:
WORKING RELATIONSHIPS
  • Frequent contact with subordinates, co-workers, consumers, State and County personnel, other authorities, community partners, and members of the public.
  • May occasionally deal with aggressive or violent individuals.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS
  • Work is performed in a variety of home, community based and office settings and involves travel. 
  • Ability to lift up to 20 pounds and a combination of sitting and standing postures may be required. 
  • Ability to minimally work 8 hours per day and 40 hours per week is required.
  • Must be able to operate and be conversant with office equipment including phone, computer, facsimile , calculator, printer and photocopier. Familiarity and capacity to work with various software applications and keyboarding proficiency are essential.
