Your Job:
Oversees day-to-day operations of the Care Management Department. Assures that appropriate resource utilization related to patient care is coupled with identified quality outcomes. Retains and develops competent staff. To assist in the planning, directing and implementing of operational, financial and personnel activities of Care Transition Navigators in the Care Management Department and provide guidance in prudent practice
Your Job Requirements:
• Master's degree from an accredited school of social work or Bachelor's degree from an accredited school of nursing
• LMSW as licensed by the Texas State Board of Social Worker Examiners or RN as licensed by the Texas State Board of Nursing
• A minimum of five years of clinical practice.
