The Health and Human Services Department is recruiting internally for a Behavioral Health Case Worker/Social Worker position.  To apply, please go to the Employment Opportunities page at https://www.governmentjobs.com/careers/trempealeau

The purpose of this position is to accept and screen referrals of persons with mental health and/or substance abuse to determine eligibility for services, develop service/crisis plans, and coordinate, monitor and adjust services. This position also oversees contracted program that provide services to persons with mental illness and/or substance abuse issues.  The work is performed under the direction of the Behavioral Health Supervisor.To perform this job successfully, an individual must be able to perform each essential function satisfactorily and in a timely manner. The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required or assigned.

Accepts and screens referrals of persons with mental health and/or substance abuse issues. Assesses environmental, psychosocial, financial, medical and other needs of referral. Conducts interviews and researches applicant records. Determines eligibility for services.  Refers to appropriate service providers.

Develops and modifies services plans and provides case management services. Monitors and evaluates on-going services for compliance with client needs and plan standards. Coordinates services.  Attends staffings at provider facilities according to service plan needs. Maintains case notes documenting client progress. Review case records to determine progress and compliance with standards, rules, regulations, and funding requirements.  

Approves placements in nursing homes and Institutions for Mental Diseases.  Completes county admission approval form 822 for nursing homes.  Attends staffings in Institutions for Mental Diseases. Assist client families with 3 party petitions for alcohol, drug, and mental health commitments.  

Schedules and facilitates meetings for reviewing client services in coordination with client, guardian, family member, guardian ad litem, and service agencies.

Advocates on behalf of clients. Contacts service agencies, care providers and other agencies on behalf of the client.

Counsels clients and client families regarding client rights, choices, financial liability, and service plans. Maintains client confidentiality and authorizes release of client information as appropriate.

Assists in completion of financial and housing resource applications. Maintains knowledge of funding source requirements including Medical Assistance programs. Coordinate with Disabilities Benefits Specialist and/or Economic Support.

Promotes services for mental health and substance abuse. May create and present public awareness information regarding Health and Human Services to medical care providers, school districts, service providers, and other agencies as directed.

Maintains knowledge of current issues, regulations, community resources and services regarding persons with mental illness, substance abuse, and other programs/services within the department. 

Authorize funding for behavioral health client services and client medications using authorization forms.  Verify accuracy of bills with supervisor and when compared with authorization including dates of service, type of service, rates and service coding.  Contact service providers to resolve any inaccuracies or discrepancies related to billing for services.

Assist with maintaining client data in the state PPS system including reports of units of services and service expenditures.

May act as Designated Coordinator for Intoxicated Driver program as appropriate and assigned by the Behavioral Health Supervisor.

Participates as a member of the Recovery Court team and assists with behavioral health assessments

Represents the Health and Human Services Department in Chapter 51 court hearings.  Works with Corporation Counsel’s Office regarding non-compliance pick up orders as necessary.  Responsible to assure evaluation for re-commitments.  Responsible to assure least restrictive environment.

Receives telephone contact evenings and/or weekends regarding Emergency placement needs and crisis consultation.  Responsible to provide proof of legal status for emergency admissions to facilities.

Coordinates services and outreach with other county agencies including Sheriff, municipal police and Corporation Counsel regarding crisis services policies and protocols.   

Recommends SSI-E certification/de-certification and completes forms.

Reports on client programming and funding issues to Behavioral Health Supervisor.  Analyzes statistical data and submits reports to Behavioral Health Supervisor as appropriate. Maintains and monitors wait list for services. Prepares a variety of correspondence, reports, and forms.

Attends the Western Region BH/SUD Manager Meeting.  Responsible to write pertinent information and share with appropriate staff.

Files and photocopies printed materials.  Fax materials as needed. 

Attends and participates in conferences, workshops, seminars and in-service training.

Maintains all licensure and certifications required for this position as applicable.

Compiles information for audit.


Ability to analyze data and information using established criteria, in order to determine consequences and to identify and select alternatives.  Ability to compare, count, differentiate, measure, copy, record and transcribe data and information.  Ability to classify, compute, tabulate, and categorize data.

Ability to counsel and mediate.  Ability to persuade, convince, and train others.  Ability to advise and provide interpretation regarding the application of policies, procedures and standards to specific situations.

Ability to utilize a variety of advisory, design data, and information such as medical reports, therapist evaluations, financial forms, service plans,  crisis plans, general assessment reports, case notes, information requests, program manuals, computer software operating manuals, most recent edition of the DSM , Physician’s Desk Reference, program policies, state statutes, non-routine correspondence, and resource directories.


Ability to communicate orally and in writing with clients, clients’ families, guardians, department personnel, other agency personnel, service providers, educators, attorneys, judges, corporation counsel, law enforcement, and the general public.

Ability to add and subtract, multiply and divide, and calculate percentages, fractions, and decimals.

Ability to use functional reasoning and apply rational judgment in performing diversified work activities.

Ability to exercise the judgment, decisiveness and creativity required in situations involving the evaluation of information against sensory and/or judgmental criteria, as opposed to criteria which are clearly measurable.


Bachelor’s Degree in Social Work, Human Services, or related field, two years case management experience preferred, or any combination of education and experience that provides equivalent knowledge, skills, and abilities.  Education, training, and/or experience working with individuals with mental illness and/or substance abuse issues preferred.  Valid motor vehicle operator’s license and/or available means of transportation required.  Social Workers are required to maintain their Wisconsin Social Work licensure or certification, to remain in the Social Worker position.  Case Workers may obtain their Wisconsin Social Work licensure or certification and be eligible to advance to a Social Worker position.
