Description
The City of Pasadena Public Health Department is currently seeking an enthusiastic and skilled Community Services Representative II to serve as a Family Health Advocate (FHA) for the Black Infant Health (BIH) program. BIH helps pregnant and parenting women develop life skills, set and attain health goals, and learn strategies for managing stress. The focus is to address the large and persistent disparities in maternal and infant health that affect the African American birthing community and align with the Pasadena Public Health Department goal of ending health inequities and achieving healthy birth outcomes for Black women. Service delivery is achieved through providing 10 prenatal and 10 postpartum group sessions covering a number of varying topics. The program also provides client-centered life planning and one-on-one case management.
IDEAL CANDIDATE
The ideal candidate will possess knowledge, understanding, and respect of the values and beliefs of African American women and the African American community, and fully embraces and supports the BIH Governing Concepts of cultural competence, client-centered, strength-based, and cognitive skill building approaches. Possesses knowledge of women’s health, including prenatal and postpartum health; life course perspective; infant behavior and development; health education including breastfeeding; nutrition and physical activity; and local community and social services. Demonstrates cultural competence and ability to operate in a culturally affirming manner; sound communication and interpersonal skills; basic counseling skills (i.e. reflecting, active listening, paraphrasing); critical thinking and problem-solving skills; and basic administrative skills (i.e. appointment scheduling, documentation, report writing, computer skills).
ABOUT THE DEPARTMENT
For over 130 years, the Pasadena Public Health Department has been responsible for helping protect, maintain, and improve the health of the Pasadena community. The City of Pasadena is one of only three cities in the State of California that maintains its own independent local health jurisdiction with responsibility for a wide variety of services that support the three core functions of assessment, policy development, and assurance. This is a limited-term, grant-funded position with funding through June 30, 2027. Many Public Health positions are funded through external grants with various time durations. VACANCY INFORMATION
There is currently one full-time, limited-term/grant-funded vacancy in the Community Health Services division of the Public Health Department. The resulting eligibility list from this recruitment may be used to fill this vacancy and similar vacancies in the future. Candidates selected for this position will serve a one-year probationary work period. Must possess a valid California Class C driver's license and comply with the City's Motor Vehicle Safety Policy during the course of employment.
Must be available to work occasional evenings and weekends.

- Works as a Case Manager to clients of the BIH Program.
- Conducts case management services that align with identified needs of each participant.
- Responsible for the development of a Life Plan that is on-going throughout the intervention.
- Completes all prenatal and postpartum assessments.
- Provides ongoing identification of specific concerns/needs and referral to services outside of BIH as needed based on Life Planning meetings.
- Maintains awareness and familiarity with local community and social services for client referrals.
- Enters data related to case management, Life Planning, etc. in a timely and accurate manner.
- Coordinates and consults with group facilitators to ensure that case management goals are linked to group session goals.
- Attends CDPH/MCAH-sponsored BIH Basic Training as well as subsequent BIH Advanced Trainings.
- Maintains confidentiality and adheres to HIPAA regulations.
- Participates in community activities to promote the BIH Program and to provide general information regarding health services available; conducts community presentations.
- Participates in case conference activities.
- Assists with prenatal and postpartum group sessions as needed.
- Proficient in the use of MS software.
- Assesses and comprehends community problems and recommends and implements appropriate courses of action.
- Assists with developing and facilitating client and community classes as needed.
- Prepares correspondence and reports and maintains data and programmatic records as necessary.
- Assist with set-up and teardown during Outreach Activities.
- May be assigned to work on special projects.

- Graduation from high school or GED equivalent.
- Two years of community service experience providing direct services to the target population and performing tasks related to the program.
- Due to the terms of the grant that funds this position, a Bachelor’s Degree from an accredited college or university program in Women/Maternal Health, Child/Infant Health, Social Work, Health Education, Human Services, or a related field is preferred and will be factored into the initial screening.
- Three years of community service experience providing direct services to the target population is preferred.
- Ability to work collaboratively within a multidisciplinary team.
