Description

Compensation & Incentives:
- $500 bonus after 50 completed visits.
- Additional earning incentives available for this service area .
- Evaluate patients' ability to safely and independently perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) within the home.
- Identify environmental, physical, and cognitive barriers affecting safety, independence, and routine function.
- Provide therapy to improve performance with self-care activities such as dressing, grooming, bathing, toileting, and meal preparation.
- Teach adaptive techniques and energy conservation strategies to promote independence in daily routines.
- Develop and implement individualized treatment plans based on patient goals and clinical findings.
- Recommend and train patients in the use of adaptive equipment and home modifications to improve safety and functional performance.
- Educate patients and caregivers on safe assistance techniques, adaptive strategies, and home safety practices.
- Collaborate with caregivers and the interdisciplinary care team to support continuity of care.
- Complete timely documentation using the agency's electronic medical record (EMR) system.
- Maintain compliance with clinical, regulatory, and agency documentation standards.
- Active New York State Occupational Therapist license.
- Experience with adult or geriatric populations preferred.
- Strong clinical reasoning and adaptability in home environments.
- Ability to work independently in a field-based role .
- Effective communication and patient engagement skills.
