Responsibilities
Healthcare shouldn’t feel like a maze. You can be the one who hands patients the map.Too many patients fall through the cracks. We’re changing that, and we need someone with heart, hustle, and a knack for connection to lead the way. Diabetes affects the whole body, and some of its most serious complications show up in places patients don’t expect: gum disease, foot ulcers, and vision loss. The good news? Many of these complications can be caught early or prevented with regular dental, podiatry, and eye care. The challenge? Juggling three specialties on top of managing diabetes is overwhelming. That’s where you come in. As our Dental Integration Coordinator, you’ll be the go-to guide helping our diabetic patients get coordinated, whole-person care across dental, podiatry, and optometry. You won’t just schedule appointments. You’ll build relationships, break down barriers, and make sure patients leaves feeling seen, heard, and supported. What You’ll Do- Be a patient’s champion. Welcome patients, learn their needs, and help them navigate care across three specialties.
- Remove roadblocks. Help patients with scheduling, insurance questions, transportation, and other barriers to care.
- Spot the gaps. Identify patients who would benefit from interdisciplinary care (for example, patients with diabetes who need dental, foot, and eye screenings).
- Educate and empower. Explain why oral, foot, and eye health matter to overall wellness, in plain, friendly language.
- Follow through. Reach out to patients who miss appointments and help them get back on track.
- Turn data into impact. Handle data collection and entry, and help your supervisor compile, analyze, and summarize information for program reports and evaluations.
- Shape the program. Contribute ideas and help plan, develop, and roll out strategies that improve how patients move through care.
- A people person who genuinely loves helping others and stays calm and kind under pressure
- Organized and detail-driven, able to juggle multiple patients and priorities without dropping the ball
