Key Responsibilities:
- Evaluate living environments for fall risks and accessibility challenges.
- Recommend modifications and adaptive equipment to improve safety.
- Assist patients with essential daily activities such as bathing, dressing, and household tasks.
- Introduce techniques to conserve energy and reduce strain.
- Develop individualized therapy programs based on each patient’s condition and home setup.
- Adjust interventions to reflect progress and changing needs.
- Instruct family members and aides on safe assistance techniques.
- Promote consistency between therapy visits.
- Maintain accurate and timely records using an electronic documentation system.
- Ensure compliance with all regulatory standards.
