Position Overview:
- 1+ year of experience.
- Medical Spanish preferred.
- Family Nurse Practitioner (FNP).
- Certified Nurse practitioner (CNP).
- Physician Assistant (PA-C).
Full-time preferred. Part-time considered.
Outpatient. No hospital rounds. No overnight calls.

Responsibilities:
- Provide comprehensive primary care to adult and geriatric patients.
- Manage chronic conditions (HTN, DM, CHF, COPD, etc.).
- Conduct annual wellness visits.
- Meet quality metrics aligned with HEDIS/value-based contracts.
- Collaborate with supervising physician and care team.
- Document efficiently in EMR.
Ideal Candidate:
- 2+ years primary care experience.
- Comfortable in value-based/capitated model.
- Strong communication skills.
- Efficient but compassionate.
- Spanish speaking preferred.
