Family Medicine Advanced Practice Provider (NP/PA)

Overview

We are seeking a dedicated Family Medicine Physician or Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to provide comprehensive, patient-centered primary care in our outpatient clinic. This role delivers high-quality care for patients of all ages, including preventive services, chronic disease management, acute care treatment, wellness counseling, and health promotion. The provider will work collaboratively with a multidisciplinary care team to ensure accessible, coordinated, and evidence-based care.

 

Responsibilities include conducting patient assessments and examinations, diagnosing and treating acute and chronic conditions, ordering and interpreting diagnostic tests, developing individualized treatment plans, prescribing and managing medications in accordance with licensure and scope of practice, providing patient education, and maintaining accurate and timely electronic health record (EHR) documentation. The ideal candidate demonstrates strong clinical judgment, excellent communication and interpersonal skills, and a commitment to quality improvement, patient satisfaction, and team-based care.

Qualifications

• MD/DO, Nurse Practitioner (NP), or Physician Assistant (PA) degree from an accredited program
• Current Georgia license and DEA registration (as applicable)
• Board Certified in Family Medicine (Physician) or national certification as an NP/PA
• Commitment to providing high-quality, patient-centered primary care
• Strong collaboration, communication, and documentation skills
• Experience in community health, FQHC, or value-based care settings preferred, but not required

Responsibilities

• Provide comprehensive primary care, including histories, exams, diagnosis, and treatment for all ages
• Manage acute conditions and chronic diseases with evidence‑based care plans
• Order, review, and interpret labs, imaging, and diagnostic tests
• Deliver preventive care including physicals, well‑child visits, women’s health, immunizations, and screenings
• Educate patients on diagnoses, medications, lifestyle changes, and follow‑up care
• Collaborate with nurses, MAs, APPs, behavioral health, and specialists to coordinate care
• Maintain accurate, timely EHR documentation and complete required forms (FMLA, disability, etc.)
• Participate in quality improvement, population health efforts, and safe medication practices
