Essential Duties and Responsibilities
- Manage, sort and distribute incoming clinical documents, whether received via fax, mail, courier or electronic transmission.
- Convert any electronically received documents to picture format and distribute
- Scan incoming clinical documents into Allscripts Input Manager within 1 day of receipt.
- Index scanned documents into patient health records accurately and efficiently
- Utilize established type codes for indexing into the health record.
- Print, fax or export patient health records in response to requests from external entities, using established Sea Mar Policies and Procedures and following state and federal law, including HIPAA.
- Maintain current and accurate documentation of any health record disclosures using established policies and procedures.
- Retrieval of purged paper charts from Sea Mar Records Center.
- Maintain immaculate documentation of all interactions regarding patient health care records.
- Check insurance eligibility for outgoing referral processing.
- Research and contact external specialists or organizations regarding outgoing referrals.
- Provide appropriate Sea Mar records for referrals to external specialists or organization, either in paper or electronic format.
- Immaculate documentation of all interactions during referral processing to an external specialist or organization.
- Maintain work area in a neat and orderly fashion
- Communicate disclosure status with external requestors
- Stay well informed and current with standard Sea Mar Health Information Policies and Procedures.
- Referral Tracking as assigned
- Provide courteous, friendly customer service to clients, patients, co-workers and community partners.
- Other duties as assigned by leadership
Conditions of Employment
Incumbent must have complete Employee Health Screening. A TB/PPD test is required annually. The
Health Information Clerk must sign an oath or permanent confidentiality covering all patient related
information and must receive a background check from the Washington State patrol.
Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
The incumbent must have analyzing ability and prioritizations skills, along with the ability to
communicate effectively and organize workload.
- Must be able to efficiently utilize keyboard.
- Good computer navigations skills are required along with familiarity with Microsoft Suite.
- Must be able to navigate Windows based programs.
Clinical
- Incumbent must have knowledge and/or training in the state and federal laws governing privacy and confidentiality.
- The incumbent must have some knowledge of medical terminology.
Language Skills
- Ability to read and interpret documents such as safety rules, operating instructions, policy and procedure manuals and reference manuals, including legal language used in court documents and state/federal regulations.
Mathematical Skills
- Incumbent must have the ability to add, subtract, multiply, and divide.
Reasoning Ability
- Incumbent must have the ability to solve practical problems.
- Must be able to interpret a variety of instructions furnished in written and oral form.
Education, Certificates, Licenses, Registrations, and Medical Screening
Education and/or Experience
High school diploma or Vocational school graduate with some medical experience preferred.
Additional Requirements
- Pre-hire and annual employee health screening required
- Annual influenza vaccine required. Only exception is for employees with a medical or religious exemption approved by Administration. Employees with an approved medical or religious exemption must wear a mask at all times during the flu season.

While performing the duties of this job, the employee is required to climb, balance, stoop, kneel,
crouch, crawl, and be able to lift up to 50 pounds. The employee is required to type and mouse for
extended periods of time, which involves the hands and arms. The employee is required to sit for
extended periods of time. Specific vision abilities required for this position include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
