Major Responsibilities  

• Manages the planning, implementation and continuous improvement of evidence-based community health programs that respond to needs identified through the Community Health Needs Assessment and align with hospital, divisional and enterprise priorities.
• Provides day-to-day leadership and coordination for program execution, including establishing workplans, aligning resources, monitoring timelines, guiding implementation activities and ensuring community health tactics are delivered in a consistent and accountable manner.
• Oversees the collection, monitoring and interpretation of program, community health assessment and outcome data to evaluate performance, identify improvement opportunities and communicate progress toward intended community health impact.
• Leads coordination with site Community Health Councils, hospital leadership, internal departments and other governance or advisory groups to promote alignment with community health strategies and support shared decision-making.
• Partners with the director to manage Community Health Assessment activities, ensuring data collection, prioritization and implementation planning processes enable each hospital to understand community needs and measure the impact of hospital interventions.
• Develops and delivers education, communication and guidance for associates, management and hospital governance on community health priorities, planned interventions and emerging issues affecting the health of the community.
• Manages processes that support accurate, timely and complete community benefit documentation for designated hospital sites, reinforcing compliance, reporting integrity and appropriate capture of community benefit activity.  
• Facilitates ongoing community benefit reporting and performance review to support accuracy, accountability and continuous improvement in hospital community benefit reporting.
• Supports trusted relationships with community leaders, community-based organizations, faith groups, public sector partners, non-Advocate health care organizations and other Advocate hospitals to advance shared community health priorities and strengthen local collaboration.
• May represent designated hospital sites in local coalitions and partnership tables, contributing a management perspective that connects community priorities, hospital strategy, program implementation and measurable outcomes.
• Communicates CHNA findings and implementation priorities to community groups, solicits input and participation, and supports shared ownership of strategies to address identified needs.

Minimum Job Requirements 

Education

• Bachelors degree in public health, epidemiology, social work, community development, public policy or a related field addressing the social drivers of health required.

Certification / Registration / License 

• N/A

Experience

• 5 years of experience addressing and improving the social drivers of health, community health strategy, population health initiatives, or related areas.

Knowledge / Skills / Abilities 

• Knowledge of community health strategy, Community Health Needs Assessment processes, community benefit requirements and social drivers of health, including current and emerging practices used by large health systems to improve community health outcomes.
• Demonstrated ability to manage community health programs from planning through implementation, including developing workplans, aligning resources, monitoring progress and supporting continuous improvement.
• Ability to lead through influence and coordinate work across hospital leaders, associates, governance groups, community councils, coalitions and external community partners to advance shared priorities.
• Strong analytical skills with the ability to collect, interpret and communicate program, community health assessment and community benefit data to support decision-making, reporting, compliance and performance improvement.
• Strong written, verbal and presentation skills, including the ability to translate complex community health information into clear guidance, education.

Physical Requirements and Working Conditions 

• ​​​ Ability to travel within the metro community. Frequent need to attend off-site meetings
• Able to work on evenings or week-ends in community.
