Position Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve strength, mobility, balance, endurance, and functional independence.
• Monitor patient progress and adjust treatment plans as needed.
• Document evaluations, treatment sessions, progress notes, and discharge recommendations accurately.
• Collaborate with nursing staff, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.
• Educate patients, families, and caregivers on exercises, safety, and functional mobility.
• Maintain compliance with applicable clinical, regulatory, and documentation requirements.

Qualifications

• Current, unrestricted Physical Therapist (PT) license in the state of Michigan.
• Graduate of an accredited Physical Therapy program.
• Strong communication, organizational, and clinical skills.
• Ability to work effectively with patients, families, and an interdisciplinary team.
• Skilled nursing facility (SNF) or geriatric rehabilitation experience preferred.
