Description
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients’ homes.
- Help patients improve mobility, strength, balance, endurance, safety, and functional independence.
- Address gait training, transfers, fall prevention, pain management, and home exercise programs.
- Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with patients, families, physicians, and members of the interdisciplinary care team.
- Coordinate with our administrative team regarding scheduling, referrals, and patient logistics.
- Build and manage your caseload based on your availability and preferred coverage area within Atlantic County.
- $55-$65 per follow-up visit.
- $75 per 4-unit evaluation.

- Graduation from an accredited Physical Therapy program.
- Reliable transportation required.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to work independently in a home-based setting.
- Home health or geriatric experience is preferred but not required.
