The newly established Medical Care at Home (MCAH) program is a cornerstone of that vision, providing safe, effective and clinically governed hospital-level care in the home. The program combines virtual care, nursing-led service delivery and multidisciplinary collaboration to support patients with a diverse range of clinical conditions and acuity levels outside traditional hospital settings.

About the Opportunity

We are seeking exceptional Medical Leads, Medical Care at Home to join St Vincent's Care at Home and play a foundational role in one of Australia's most innovative healthcare initiatives.

Based in Brisbane, we have both Permanent Full-Time and Permanent Part-Time opportunities available.

These senior clinical leadership roles will operate across a 7-day roster as required (inclusive of an on-call roster), supporting the delivery of safe, responsive and clinically governed Medical Care at Home services while ensuring medical oversight for patients receiving hospital-level care beyond traditional hospital settings.

Operating from the St Vincent's Care Coordination Centre, you will work closely with nursing, operational and executive leaders to establish clinical credibility, foster stakeholder engagement and develop innovative care pathways that safely bridge the gap between hospital and home.

You will provide medical governance for a nurse-led model of care, support the rollout and expansion of services across Queensland and contribute to the national growth of Medical Care at Home services across New South Wales and Victoria.

This role will be instrumental in building clinical trust in higher-acuity virtual care, partnering closely with Visiting Medical Officers (VMOs), hospital clinicians and healthcare leaders across St Vincent's hospitals in Brisbane, Toowoomba, Sydney and Melbourne, and other locations as required.

As the service grows, you will become part of a team of Medical Leads providing national coverage and helping guide the future direction of Medical Care at Home across Australia.

Key Responsibilities

Clinical Leadership & Medical Governance

  • Provide medical oversight, governance and clinical leadership for the Medical Care at Home program.

  • Deliver virtual medical reviews, telehealth consultations and clinical escalation support for a nurse-led model of care.

  • Admit patients into the service and oversee ongoing clinical management.

  • Conduct clinical assessments, diagnoses and treatment planning.

  • Order, review and interpret diagnostic investigations and communicate results appropriately.

  • Support management of clinical deterioration, escalation pathways and emergency responses.

  • Lead multidisciplinary care planning, virtual ward rounds and complex case reviews.

  • Coordinate prescriptions, specialist consultations, discharge planning and continuity of care.

  • Maintain comprehensive and timely clinical documentation.

  • Participate in clinical governance activities, Medical Advisory Committees (MACs), quality reviews and operational forums.

Service Growth & Innovation

  • Develop, implement and continuously refine clinical pathways that support hospital substitution and home-based care.

  • Expand patient cohorts and clinical services through safe and evidence-based pathway development.

  • Support innovation in virtual care delivery and connected health services.

  • Contribute to service monitoring, evaluation, research and quality improvement initiatives.

  • Champion contemporary models of care that improve patient outcomes and healthcare sustainability.

Stakeholder Engagement & Change Leadership

  • Build trusted relationships with VMOs, clinicians, executives and healthcare partners.

  • Drive engagement and service adoption across St Vincent's hospitals and clinical teams.

  • Support local change management initiatives that strengthen confidence in virtual and home-based care.

  • Act as a clinical ambassador for Medical Care at Home services across the organisation.

  • Collaborate effectively within a dual reporting model, supporting both operational and clinical priorities.

National Collaboration

  • Contribute to the national strategy and expansion of Medical Care at Home services.

  • Support growth opportunities across Queensland, New South Wales and Victoria.

  • Travel as required to support stakeholder engagement, implementation activities and service development initiatives.

  • Work collaboratively within a national network of clinical and operational leaders.

About You

You are a highly respected medical practitioner who combines strong clinical expertise with a passion for innovation, leadership and healthcare transformation.

You are excited by the opportunity to challenge traditional models of care, build clinician confidence in virtual healthcare and help create new pathways that improve patient outcomes while reducing unnecessary hospital admissions and length of stay.

You Will Bring

  • Medical degree with current unrestricted medical registration.

  • Fellowship with RACGP or an equivalent specialist qualification.

  • Demonstrated experience as a Medical Practitioner or General Practitioner.

  • Extensive experience in acute clinical care, Hospital in the Home (HITH), virtual wards, telehealth or related models of care.

  • Demonstrated experience using Electronic Medical Records (EMRs), patient administration and care management systems, including digital clinical documentation, electronic prescribing platforms and other technology-enabled healthcare solutions.

  • Proven ability to provide medical oversight, clinical escalation support and remote diagnostics within multidisciplinary healthcare settings.

  • Demonstrated expertise in clinical governance, patient safety and quality improvement.

  • Experience leading complex case reviews and multidisciplinary team discussions.

  • Outstanding stakeholder engagement and relationship management capabilities.

  • Proven ability to influence and gain buy-in from VMOs, clinicians and healthcare leaders.

  • Strong communication, leadership and change management skills.

  • High adaptability and willingness to work across both virtual and face-to-face clinical environments.

  • Strong commitment to innovation, evidence-based care and continuous improvement.

Highly Regarded

  • Experience leading healthcare transformation initiatives.

  • Experience implementing virtual care, hospital substitution or home-based healthcare models.

  • Previous experience within telehealth, command centre or care coordination environments.

  • Experience designing and implementing clinical pathways and service models.

  • Understanding of private healthcare systems and funding models.
