What You’ll Do

• Provide skilled nursing care in accordance with physician orders and care plans
• Review and follow the patient Plan of Care before each visit
• Monitor patient conditions and communicate changes with the RN Case Manager and care team
• Educate patients and caregivers on treatment plans and self-care techniques
• Support patient safety initiatives and identify opportunities to prevent avoidable hospitalizations
• Administer treatments and document care accurately and timely
• Collaborate with physicians and interdisciplinary team members to ensure quality patient care
• Deliver compassionate care and exceptional service to patients and families

What You’ll Need

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• Current Indiana LPN license
• Minimum of 1 year of nursing experience in a clinical care setting
• Home Health or Hospice experience preferred
• Ability and willingness to travel within assigned territory
• Valid driver's license, current auto insurance, and reliable transportation
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
