Admissions Assistant

Pay: $19.00 - $26.00 per hour

Napa Post Acute is seeking a motivated, organized, and proactive Admissions Assistant to join our team!

Our Admissions Department is fast-paced and constantly evolving. We're looking for someone who can confidently manage multiple priorities, think independently, anticipate challenges, and find creative solutions.

Key Responsibilities

• Monitor and review incoming referrals, ensuring timely and accurate responses.
• Make appropriate admission decisions using established facility guidelines and recognize when escalation is necessary.
• Maintain consistent communication with the Admissions Director.
• Process incoming admissions, including PointClickCare (PCC) entry, room coordination, and required documentation.
• Complete admission packets and maintain timely, accurate documentation.
• Anticipate admission barriers and proactively identify solutions to maximize appropriate placement opportunities.
• Coordinate with nursing, housekeeping, and other departments to ensure smooth admissions.
• Conduct facility tours when the Admissions Director is unavailable.
• Independently balance referrals, admissions, and packet completion throughout the day.

• Excellent follow-through, accountability, and attention to detail.
• Ability to work independently without constant direction.
• Comfortable working in a fast-paced, high-volume environment.
• Computer skills are required.
• Previous skilled nursing or healthcare admissions experience is a plus.
• Experience with PCC and hospital referral platforms is a plus.

Schedule

This is a full-time position with a 10:00 AM start time. Weekend availability is required, with a consistent schedule to be determined.
