Description
This position tracks data including caseloads, intakes, MCO enrollment, initial assessments, annual reassessments, and APS screenings and assessments to evaluate needs. This position identifies service gaps and collaborates with providers for service development. This position supervises case managers, care coordinators, APS workers, and support staff assigned to program areas providing consultation, individual meetings, and annual evaluations. This position also prepares and leads meetings for adult protection and aging services program areas, has knowledge of the legal process and assumes the lead role for DHS assessment and case management appeals, and acts as the point of data release for assigned program area, reviewing necessary statute and consulting as needed. Additionally, this position demonstrates ability to communicate effectively with clients, staff, and the public, and collaborates with other Health and Human Services units and counties.
- Acts as the point of data release for program areas, reviews statute and consults with Human Resources, Health and Human Services Director, and County attorney as needed
- Co-chairs the Guardianship Committee, and works with the Disability Services Supervisor and County Attorney to ensure procedures follow legislative statute. Participates in determinations of county funded guardianship/conservatorship requests and hearings as requested
- Collects and maintains APS data, participates in intake screenings and determinations, reviews APS documentation, tracks APS grant allocations, consults with APS partners at DHS, and participates in regional meetings
- Collects and maintains working data on monthly client reassessments and client lists. Reviews monthly enrollment, MCO sites, and attends MCO trainings. Assists with processing SNF pre-admission screenings
- Forecasts budget, services allocation, manages ongoing grant and budget expenditures, based on caseload trends and changes in programs
- Oversees and monitors program budgets. Authorizes programmatic expenditures
- Reviews case records, time reporting, and program data to ensure that agency, state, and managed care organization policies, rules, and regulations are adhered to. Coordinates MCO annual audits with follow-up on corrective action plans as needed
- Selects and trains new staff with monitoring of progress and evaluation of performance. Provides coaching and mentoring
- Supervises case managers, and a case aide assigned to program areas. Provides case consultation, feedback, monthly one-on-one (1:1) meetings, and evaluations using effective communication. Intervenes and assists staff to resolve client situations
- Supervises Home and Community Based Services programs and assessments serving clients sixty-five (65) and older, processes for three (3) managed care organizations, and Adult Protection. Reviews and interprets policy, procedures, and legislation. Performs program analysis and needs assessment
- Takes the lead role in sixty-five (65) and older assessment and case management appeals
- Works with providers on service development and delivery. Represents the county or department in community, provider, and lead agency meetings
- Valid MN Driver’s License, and
- A bachelor's degree in one (1) of the behavioral sciences or related fields including, but not limited to, social work, psychology, or nursing from an accredited college or university, and
- Three (3) years of relevant experience

- Skilled in using a computer, Microsoft Office and related software applications
