$25.67 - $37.70

Job Description:

Purpose of Job:
Improve the overall health of the communities we serve by expanding access for maternity and young pediatric populations to quality health and wellness services as follows:

Essential Duties and Responsibilities:

  • Conducts intakes and assessments of client needs in accordance with First Steps Program guidelines, which includes the comprehensive and ongoing identification of health care, social, and education needs involving high-risk care. 
  • Documents progress and follow up in compliance with First Steps billing guidelines. 
  • Advocates and refers patients to appropriate community resources. 
  • Counsels and aids patients and their families to understand healthcare recommendations and options. 
  • Assesses and assists with non-clinical problems interfering with effective patient care under supervision. 
  • Works collaboratively with patients, CHAS Health multidisciplinary teams, and outside entities to ensure healthy birth outcomes and infant support. 
  • Teaches maternal and infant care related to individual and community welfare as required.
  • Assists in special studies, research programs, and resource development.
  • Participates in community outreach activities as directed.
  • Provides and/or facilitates at-home visits for patients as needed.
  • Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values.  

Qualifications:
Education/Experience: Minimum of two years of health and/or social services experience required. Bachelor’s degree in a social service-related field, such as social work, behavioral sciences, psychology, child development, or mental health required.
     
Skills: Excellent oral and written communication skills required. Compassion for patients required. Computer skills required.  Ability to work under tight time constraints, handle high pressure and changing priorities required. Self-motivated with ability to work independently and as part of a team required. Valid driver’s license and insurance required. BLS (CPR/AED) required. Commitment to supporting a safe and respectful environment is required.

Physical Demands:
Must be able to move around the facility between one-third and two-thirds of the day, as well as sit at a workstation and in patient rooms. Using hands occurs over two-thirds of the day, while reaching with arms occurs less than one-third of the day. Communicating is also required over two-thirds of the day, while climbing, stooping, or crawling is minimal.  Lifting up to 10 lbs. occurs about half the time, while up to and above 25 lbs. occurs only occasionally. Driving frequently is required for onsite visits.
